• REQUEST METHODS

To receive your credit report, you may either submit requests to the following agencies by the following methods:

• INFORMATION NEEDED

Please be prepared to provide the following information:

1. Your FULL name (including middle initial and suffixes)
2. Spouse’s name (if applicable)
3. Your addresses for the last five years, including your current address
4. Your Social Security number
5. Your date of birth
6. Name and address of your current employer
7. Home phone number

If you are making a request by mail, please also provide the following:

1. Photocopy of a utility bill and your drivers license verifying your current address (other items that show proof of address also are acceptable).
2. Sign the written request.

• COSTS

The credit report will be free if, within the last 60 days, you have been denied credit, insurance or employment based on your credit report or if you are a victim of credit fraud.   Otherwise, the charge will be $8.00 for Experian and $8.50 for Equifax and Trans Union.

• TIME

You should receive your credit report in 5-10 days after the submission of your request.
